What to do if…
you develop sudden severe heartburn or chest pain after swallowing a pill

Short answer

If you have severe chest pain, trouble breathing, fainting, heavy sweating, nausea, or pain spreading to your arm/neck/jaw/back, call 911 now.

Do not do these things

  • Do not take another dose of the same pill “to fix it” or “test it”.
  • Do not lie down flat (unless you feel faint—then lie on your side and call 911).
  • Do not force it down with more pills, dry food, or large gulps.
  • Do not try to make yourself vomit.
  • Do not drive yourself to the ER if you feel very unwell or think it could be heart-related—use 911.

What to do now

  1. Decide if this is an emergency (when in doubt, treat it as one).
    Call 911 immediately if you have:
    • severe chest pain/pressure, or chest pain that is not easing
    • difficulty breathing, new confusion, fainting/collapse, or you’re hard to wake
    • sweating, nausea/vomiting with chest discomfort
    • pain spreading to one/both arms, neck, jaw, back, or upper stomach
    • you cannot swallow saliva (drooling), are choking/wheezing, or your lips/face/tongue are swelling
    • you vomit blood or material that looks like coffee grounds, or you pass black/tarry stools
  2. Sit upright and stay still while help is arranged.
    Sitting upright can reduce reflux and may reduce irritation if a pill is stuck. Keep your phone close; if you’re alone, unlock the door if you can.
  3. Stop further irritation (don’t try to “wash it down”).
    Don’t take any more of the suspected pill. If you can swallow comfortably, take tiny sips of water—don’t force it if swallowing is painful, you’re drooling, or you feel like you may choke.
  4. If it’s not an emergency but symptoms are persistent or painful, get same-day medical care.
    Go to an urgent care or ER if pain is significant, you have pain when swallowing, you feel like the pill is stuck, or symptoms are not settling.
  5. Use Poison Control for medication-related guidance (when you are stable).
    If you are not having emergency symptoms, call Poison Control at 1-800-222-1222 for free, confidential advice. If severe symptoms develop at any time, switch to 911.
  6. Collect key info for clinicians (fast and useful).
    Keep the medication bottle/packaging. Note the pill name, dose, when you swallowed it, whether it was with water/food, and what it felt like (burning vs pressure; pain when swallowing; “stuck” sensation).

What can wait

  • You do not need to figure out right now whether this is heartburn, reflux, or an esophageal injury—triage and safety first.
  • You do not need to decide today whether to permanently stop the medication—just pause the next dose until you’ve been advised.
  • You do not need to search for home “fixes” in the moment; avoid anything that increases choking risk or irritation.

Important reassurance

Chest discomfort can feel scary and can be hard to interpret—many people can’t tell heartburn from something more serious in the moment. Choosing urgent evaluation when symptoms are severe or unusual is a safe, reasonable step.
