What to do if…
you develop sudden spinning dizziness and you have to lie down
Short answer
Lie down somewhere safe and do a quick stroke check. If you have any stroke warning signs (face/arm weakness, speech trouble, new confusion, sudden vision changes, severe headache, or sudden trouble walking/balance), call 911 now.
Do not do these things
- Do not try to walk it off or stand unassisted (falls are a major risk).
- Do not drive yourself to urgent care or the ER.
- Do not drink alcohol or take extra sedating medicines to “calm” the spinning; only take medicines as directed/prescribed, and do not take someone else’s medication.
- Do not get into a bath/shower alone or use stairs while it’s spinning.
- Do not ignore dizziness with new one-sided weakness/numbness, slurred speech, new confusion, new vision loss/double vision, or a sudden severe headache.
What to do now
- Make it physically safe first.
Lie flat (or slightly propped if that reduces nausea). Keep your head as still as you can. If you might vomit, roll onto your side to protect your airway. - Do a 30–60 second emergency screen (FAST + walking/vision).
- Face drooping?
- Arm weakness on one side?
- Speech difficulty/confusion?
Also watch for: sudden trouble walking/standing, severe loss of coordination, or sudden vision loss/double vision.
- If any stroke warning sign is present (or you’re unsure), call 911.
Tell the dispatcher: “Sudden onset spinning dizziness/vertigo, started at [time], I had to lie down.” - Even if FAST is normal, treat it as an emergency if any of these are present:
- Ongoing vomiting you can’t control
- Chest pain, trouble breathing, fainting/seizure, or a racing/irregular heartbeat
- Sudden hearing change/loss, or sudden double vision/major vision change
- You cannot stand even while holding onto something sturdy
In these cases: call 911 or have someone take you to the ER (do not drive yourself).
- If it’s severe/new but none of the above emergency triggers apply, get same-day medical evaluation.
If you can’t safely sit/stand or symptoms are escalating, the ER is appropriate. If you’re stable and improving, an urgent care/clinic may advise next steps—avoid driving yourself. - Get another person involved if possible.
Call/text someone to stay with you, help you move safely, and watch for worsening symptoms. - Write down 4 details for clinicians (phone notes is fine).
- Exact time it started.
- What you were doing (rolled in bed? stood up? after an illness?).
- Ear symptoms (hearing change, ringing, ear pain).
- Any neurological symptoms (weakness, numbness, speech/vision trouble).
- If you can swallow and you’re not actively vomiting: take small sips of water.
If you’re actively vomiting or very drowsy, don’t force fluids—focus on safe positioning and getting urgent care.
What can wait
- You don’t need to figure out the cause right now (different conditions can feel the same in the moment).
- You don’t need to try online “fixes” or head manoeuvres during a severe first episode.
- You don’t need to decide about work, driving, or long-term treatment until you’re assessed or clearly improving.
Important reassurance
Spinning vertigo can feel intense and frightening, and needing to lie down is common. Many causes are treatable, but because stroke can present with sudden dizziness and balance problems, ruling out stroke and other emergencies first is the safest approach.
